Scheduling

Scheduling is encouraged to be done through our online scheduling platform, via phone, or email. All appointments are to be scheduled within normal business hours.

Cancellation & No-Show Fees

We realize that life is not always predictable and understand that there may be times where you must miss an appointment due to circumstances beyond your control, whether emergencies or obligations for work or family. If you cannot make your appointment, we ask that you contact the office via phone or email at least 24 hours in advance to cancel or reschedule your appointment.
Clients who fail to show up for their appointments or cancel less than 24 hours in advance will be charged a $50.00 no show fee to their account. This fee may be required to be paid prior to scheduling further appointments within the practice. Multiple no shows or canceled appointments may lead to dismissal from the practice.

Non-Participating Provider Consent

Capital Physical Health Services, LLC operates as a cash-based chiropractic wellness practice. Neither Capital Physical Health Services, LLC nor the Doctor participates in any health insurance plan or HMO plans or panels and provides Chiropractic and wellness services that are not covered under Medicare guidelines. As a client of the practice, you acknowledge this and are solely responsible for all services rendered by the doctor. Executive Healthcare Services Affiliation

Capital Physical Health Services (CPHS) and Dr. Darrel Asuncion is an independent Chiropractic and wellness service provider, operating within the offices of Executive Healthcare Services (EHS), a concierge primary care provider. As distinctive clinical practices, membership with CPHS or receipt of services by Dr. Asuncion does not initiate or qualify for membership at EHS or institute a physician – patient relationship with any individual clinician in the EHS practice. Conversely, membership at EHS does not institute care with CPHS or launch a physician – patient relationship with Dr. Asuncion. EHS and its clinicians are not liable for services rendered by CPHS or by Dr. Asuncion, nor is CPHS or Dr. Asuncion liable for services rendered by EHS or any of its clinicians.
